在臨床精神科實務中,職能治療師運用有目的的活動(purposeful activities)來評估、促進、恢復、或維持病患參與職能(occupations)的能力。其中,創造性藝術活動可說是一種以行動為導向的介入方式,當精神病患的表達能力較差,或當其言詞無法盡其意時,可以轉而藉助圖像與符號來傳達自己的感覺、增進個人的成長、復健以及變型(transformation);此外,藝術創作也是一個避開恐怖經驗的避難所,可充當病患內心世界與外在真實生活間的聯繫管道。本文除介紹精神疾病患者之創造性及創作的過程外,再佐以筆者於臨床實務中的三種表達性藝術活動為例,並針對不諳藝術背景之職能治療師提出如何運用藝術媒材之建議,期能提供臨床職能治療師之參考。
In the clinical practice of psychiatric wards, occupational therapists usually exploit purposeful activities to evaluate, enhance, restore, or maintain patients' performance of occupations. Among them, creative art activities belong to a category of action-oriented intervention through which psychiatric patients who are defect in verbal expression or those who cannot completely present themselves verbally are able to display their thoughts and feelings with images and symbols, so as to promote personal growth, rehabilitation and transformation. In addition, art performance is figuratively a harbor of refuge in which clients can be protected from the roaring waves of unpleasant experience and memory, as well as a communication channel between patients' inner lives and the realistic world. In this article, psychiatric patients' performance and process of creative art are described and explained with examples of three expressive art activities used in the author's clinical practice. Suggestions of how to utilize various art media are also made for clinical occupational therapists with little or no art background.
