目的:近年來有許多研究提出全身震動療法對於中風個案的潛在效益,本文藉由系統性文獻回顧以及後設分析了解全身震動療法對於中風個案之動作、平衡與功能性移行能力相關的治療成效。方法:經由 PubmedScopusCINAHLCochrane Library、華藝線上圖書館進行文獻搜尋,搜尋期間為 2003 1 月至 2023 1 月,後設分析使用 review manager version 5.4 進行,分析坐站起走測試、柏格氏平衡量表、十公尺行走測試、六分鐘行走測試及傅格梅爾評估量表共五個變項。結果: 27 篇文獻符合收入條件,其中有 12 篇文章在平衡能力、肌力、痙攣、動作能力、日常生活自理功能,以及功能性移行能力皆具有顯著的組間差異,後設分析結果顯示,以柏格氏平衡量表作為結果測量時,全身震動治療組的治療效果優於控制組 (SMD = 0.64, P = 0.007);若以傅格梅爾評估量表與坐站起走測試作為結果測量,則組間未達顯著差異,但介入效果傾向實驗組的進步較多 (SMD = 0.26-0.41, P = 0.09-0.31);但若以六分鐘行走測試作為結果測量時,介入結果則偏向控制組進步較多 (SMD = -0.23, P = 0.14);而十公尺行走測試則未有明確療效 (SMD = -0.09-0.01, P = 0.61-0.93)。結論:本研究結果顯示全身震動療法可以顯著改善中風個案的平衡能力,而針對功能性移行能力、動作恢復能力則未有顯著的成效。由於結果測量的工具差異性較高,故可納入後設分析之研究較為侷限,因此未來仍需要有較多的研究去證實其療效。由於全身震動療法具有被動、安全的特性,可以作為中風個案的輔助治療。

Background: Stroke is a neurological disorder which combined with sensory, motor, cognitive deficits and many other symptoms. Researchers have suggested that whole body vibration (WBV) could improve motor function, balance and functional mobility for people with stroke. This study was to examine the effectiveness of whole body vibration in people with stroke by systematic-review and meta-analysis. Methods: The following databases, including MEDLINE, PubMed, CINAHL, Cochrane Library, and Ariti Library, were used to search for articles. Articles published between 2003 and 2023 January were included. Review manager Version 5.4 was used for meta-analysis to analyze the results of Timed Up and Go Test, Berg Balance Scale, 10-Meter Walk Test, 6-Minute Walking Test and Fugl-Meyer Assessment. Results: A total of 27 articles met inclusion criteria and were included in the systematic review and 20 of them were included for meta-analysis. The results of meta-analysis indicated that the effect of WBV was significantly superior to the control group in the Berg Balance Scale (SMD = 0.64, P = 0.007). The results tended to favor the experimental group in Fugl-Meyer Assessment (SMD = 0.41, P = 0.09) and Timed up and go test (SMD = 0.26, P = 0.31). However, the results tended to favor the control group in 6-Minute Walking Test (SMD = -0.23, P = 0.14). Discussion: Current findings suggested that WBV was superior to the control group in improving balance but not improving motor function and functional mobility. However, the effect of WBV for stroke was inconsistent among studies because most studies used diverse intervention protocols and different outcome measures. Conclusion: WBV could be a potential treatment for people with stroke, especially in balance function; however, it required further studies to examine the effectiveness.
