西元 2015 年 6 月 27 日於新北市八仙水上樂園發生塵爆事件而造成大量的燒燙傷個 案,緊急醫療期後個案開始面臨復健的問題。為能執行大量個案復健業務且促進復健之動 機,本團隊首度以團體治療的形式執行復健計畫。本團隊將團體復健時程整合成三季,三 ~四個月為一季。文中紀錄各季的復健目標與執行方式。我們以健康照護矩陣 (Health care matrix) 檢視團體治療之品質,在第一季呈現有效率的處置,也發現部分成員認為團體治 療因個案組成複雜而無法提供及時的服務,在第三季末時,因懸殊的醫病比而導致人力資 源的浪費。另外,我們記錄本次燒燙傷團體治療的多種面向,如全民健康保險 (National Health Insurance, NHI) 收益、照護比、成員心理支持層面、工作時間、團體動力層面。 發現團體治療能夠減少治療師的工作負擔、縮減工作時數、提升個案的治療次數、平均健 保收入,團體治療也能提供團體動力,增加個案復健動機並使照顧者與個案之間形成支持 性聯絡網。惟團體治療仍有限制如需要足夠的空間、適當的人力配置、治療品質較不穩定 等,仍需依照各單位實際情況調整。結論而言從治療人力運用、團體動力、資訊交換、心 理支持、團體治療效能等都有正面意義,希望本次的團體治療經驗可以作為臨床人員面臨 大量相同診斷之個案時的參考。
On 27 June 2015, a dust explosion accident happened at the Formosa Fun Coast, New Taipei city. It caused a lot of burn injury patients at that time. To treat a large number of patients and provide emotional support, we organized a group therapy model. It was divided into threesections (3~4 month/section). We recorded our goal and plan of each section. Then, we use ahealthcare matrix to assess the quality of this group therapy model. The result shows that wecould not offer on time service due to the different injury pattern between each member in rstsection. On the other hand, we found a wide range of therapist-patient ratios in group therapy could cause the waste of the human resources in section 3. We also analyze the relationship between the payment of NHI and therapist-patient ratios, it shows several positive advantages such as reducing the workload, increasing the times of therapy, information exchange and it also created the corrective emotional experience, group dynamic supplication and supportive social network. It still has several restricts like space requirement , human resources, unstable treatment quality. So it is necessary to evaluate the resource in different unit . We would like to share this experience as reference to every staff who is going to organize a group for the patients with the same diagnosis.
