目的:世界衛生組織於2007年出版「國際健康功能與身心障礙分類模式-兒童與青少年版」(The International Classification of Functioning, Disability, and Health- Children and Youth Version, ICF-CY)提供醫療專業一致且標準化的語言及架構描述個案健康及健康相關狀態。本篇報告以此架構及內容分析一位五歲五個月大自閉症男童之臨床問題。個案資料:個案目前就讀普幼中班,指令配合度佳,課業學習及生活自理能力沒有特別問題,家長主訴為社交互動能力弱,與案弟及同學的相處技巧差,經過治療介入,目前已有明顯進步。以ICF-CY模式進行資料蒐集,並就其健康狀態、身體功能及構造、活動及參與、以及環境因素進行編碼及分析。結語:ICF-CY的架構與職能治療所強調的主動參與及著重環境的理念一致,因此在分析時對職能治療師來說,能將臨床推理流程具像化,並更能與團隊中其他專業人員溝通,提升服務品質,是一個值得學習應用的架構。
Object: World Health Organization (WHO) have published "International Classification of Functioning, Disability, and Health and the version for children and youth (ICF-CY)" to provide a consistent and standardized language and structure for the medical profession to describe health and health-related state of the client in 2007. We reported a five-year old autistic boy by using the model of ICF-CY. The client studied in a general kindergarten, with good cooperation, and had no special problem in learning and self-care abilities. His parents complained of his poor social-interaction ability, and poor interpersonal skills with his younger brother and classmates. During treatment, his abilities had been obviously improved. We used "ICF-CY model" as a framework to collect, code, and analyze data. The concept of ICF-CY model matches with the philosophy of occupational therapy; therefore, the concretization of the clinical reasoning process can help occupational therapists to communicate with the other professionals. The ICF-CY model is worthy of further learning and application.
